The visual layer displayed streaming text tokens smoothly, but the accessibility layer treated each chunk as a new assertive announcement, causing previous speech to be aborted. This mismatch created focus order issues, including a copy button appearing prematurely and stealing tab focus from the text composer. The bug was identified through logging focus events and live-region mutations rather than visual inspection alone.
